| Applicants will be expected to attend an interview with Foundation representatives in July of the year for which assistance is requested. Interviews are informal and are held to allow the interviewers to get to know the applicant, and for the applicant to ask questions. . |

| Applicants will be required to provide the following before any approved financial
assistance can be distributed : |

Copies of the applicant's IRS Form 1040 (federal income tax return) for the year ended on the most recent December 31st.

A copy of the applicant's tuition bill showing the financial aid that has been awarded for the coming academic year. If that information does not appear on the tuition bill, some other evidence of financial aid must be provided, along with a copy of the tuition bill (e.g. notification letter from PHEAA). The tuition bill should show all of the costs of attending the school, including tuition, room, board and fees. Click here to download an example of this form in PDF format.

A recommendation from the individual in the Harrisburg School District to whom you are responsible.
